The Florida Office of Insurance Regulation (OIR) has revised the Uniform Mitigation Verification Inspection Form. The revised form, OIR-B1-1802 (Rev. 04/26), will be effective April 1, 2026. Inspections conducted on or after April 1, 2026, must use the revised form.
Notes:
- If no changes have been made to the structure, Citizens will continue to accept the previous version of the form, OIR-B1-1802 (Rev. 01/12), if it was completed within five years prior to April 1, 2026.
- Performance violations may be issued for submitting an older form.
- Agents should be aware of changes made to the Uniform Mitigation Verification Inspection Form regarding Commercial Residential Type I buildings.
Citizens has created a new Wind Loss Mitigation Guide for the OIR-B1-1802 (Rev. 04/26) to assist agents with navigating the changes and new features added to the form.
Existing Feature Changes
The new form has updates to the current mitigation features below. Agents are encouraged to review the updated form in its entirety to ensure familiarity with all revisions:
- Building Code
- Roof covering
- Roof deck attachment
- Roof to wall attachment
- Secondary water resistance (SWR)
- Opening protection
New Features
Along with other revisions, the OIR has added the following new features to the form.
- FORTIFIED Home™ – Roof, Silver, and Gold certificates will be eligible for specific mitigation features on the new form.
- Region – location is based on design windspeed.
- Roof Slope* – a single-family home with multiple slopes will need to be indicated.
Notes:
- All fields on the form must be completed by the inspector.
- The agent should review to confirm that the inspection is complete and all required photographs are attached prior to submission.
- These additional changes and new features will not result in any difference to the credits available and applied at this time.
- No fields will be added or changed in Clearinghouse Powered by EZLynx® or PolicyCenter® at this time.
