New Clearinghouse Form Required for New Business– Effective 9/1/25

The Office of Insurance Regulation (OIR) approved a new form, Clearinghouse Program – Know Your Options, required for new-business submissions effective on or after September 1, 2025, for policy types submitted through Clearinghouse Powered by EZLynx®. This form brings additional awareness to the policyholder, allowing them to acknowledge they were informed of all carriers offering coverage. The form will also list all offers of coverage with carriers for which their agent is appointed, and it must be signed by both the agent and the policyholder.

PolicyCenter® will be updated to include the form when agents print the new-business applications beginning August 1, 2025, and the form will be listed as a required document for HO-3, HO-6, DP-1-D, and DP-3-D policies with an effective date on or after September 1, 2025.

The Clearinghouse Program — Know Your Options form has been added to the applicable required document guides.

  • Personal > PR-M to locate the applicable Required Document Guide in the Spotlight section on the right-hand side
  • Training > Webinar Recordings for webinars to assist agents with submitting new-business risks through EZLynx to verify eligibility for a Citizens application 
  • Training > On Demand Education to view the Citizens Clearinghouse Powered by EZLynx submission demonstration video
  • Training > Personal Job Aids, and look under Clearinghouse to review EZLynx & Citizens Resource Hub and related job aids and submission forms
